Accident Report Form

If you are involved in a motor vehicle accident, and the damage is over $1,000.00 or there is personal injury, you must file an accident report within five (5) working days of the day of the accident.

  • One accident report must go the the Massachusetts Registry of Motor Vehicles.
  • One accident report must go to the Police Department that has locational jurisdiction.
  • One accident report must go to YOUR own insurance company.
